Please understand that every effort will be made to accommodate your preferences, however, changes may need to be made “in the moment” to ensure safe adult-to-child ratios. Thank you in advance for being flexible and willing to serve wherever the need is greatest!

Nursery | Toddlers (infants up to age 2 or 3 and not enrolled in PreSchool)

PreSchool (children who are potty-trained, enrolled in preschool and able to sit at a table for activities, snacks, etc.)

Pre-K | Kindergarten (children enrolled in PreK or Kindergarten)

Elementary (children in grades 1st through 5th)

Middle School (kids in grades 6th through 8th)

Check-in/Security (welcome new families and get them set up in Planning Center; assist volunteers & existing families with check-in, ensuring all kiddos are checked into the correct class)

How often? *

How often are you willing to serve? We would prefer that all volunteers serve at least 2x/month or every other week. This gives you and the kids a chance to really connect, keeps you up to date on the curriculum and offers some stability and consistency for the kids. If you have specific Sundays (i.e. 1st & 3rd) you're available, please indicate that in the next section.

I authorize the release of the information contained in this application to any ministry at Followers Church in which I seek to volunteer. In consideration of the receipt and evaluation of this application by Followers Church, I hereby release any individual representative of Followers, including record custodians, both collectively and individually, from any and all liability for damages of whatever kind or nature which may at any time result to me, my heirs, or family, on account of compliance or any attempts to comply, with this authorization.

Should my application be accepted, I agree to refrain from unscriptural conduct in the performance of my services on behalf of Followers Church.

I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND SIGN THIS RELEASE AS MY OWN FREE ACT.

Volunteer Agreement *

I have prayerfully considered serving in FCKids. Realizing the value of this ministry, I am committed to serve these children on a regular basis.

By checking this box, I can be counted on to:
• Live a life devoted to Jesus in prayer, reading and obeying God’s Word.
• Read and abide by the Discipline and Intervention Policy (will be sent upon receipt of completed application)
• Accept or Decline service requests in a timely manner and submit block-out dates (planned weeks off) as early as possible (preferably by the 15th the prior month)
• Be present at every service for which I am scheduled, with the exception of illness or emergency
• If unable to serve due to illness or emergency, notify the kids pastor or director as early as possible
• Arrive 20 minutes prior to the service ready to serve
• Follow the curriculum as prepared
• Notify the kids pastor of any safety or behavior concerns right away

Consent to Background Check *

By checking this box, I understand that passing an electronic background check through Checkr, a 3rd party service, is a requirement to serve with children in any capacity and agree to provide information necessary to complete this.

*This request will be sent to you via email as soon as your application is submitted and must be completed electronically by you within 3 business days of receipt. Followers Church receives a “clear” or “question” response. If a background check comes back with a “question,” you’ll be contacted by a Followers Pastor for explanation/clarification before being approved to serve with kiddos.

Commitment to Growth *

As volunteers, you represent Followers Church to new families, your friends and family, your co-workers, etc. We think it's important that all volunteers understand who we are and what we believe and are actively working to grow in their walk with Jesus. That's why we expect all volunteers to complete the OnRamp (101) and Merge (201) classes, attend regular services in addition to serving (i.e. serve one, attend one) and to be involved in a small group. By checking this box, you acknowledge that you understand these expectations.
