Statement of Health

Travel insurance is purchased for each team member, however, the travel insurance is intended to pick up where your personal health insurance leaves off. A Statement of Health form will be required by each team member shortly before departure. The form will include your personal medical insurance information, prescription medication list, and doctors names and contact info. To ensure team member privacy, the forms will be placed in sealed envelopes and only opened in the event of a medical emergency.

There may be regulations around carrying certain medications through international customs. You are responsible for ensuring you have enough medication to last the duration of the trip as well as ensuring the medication is packed and transported according to TSA and customs regulations.

Illness and Injury Release *

Travel to a developing country could involve inherent risks and dangers, both expected and unexpected, that may result in serious bodily injury of various type, kind, and nature, up to and including paralysis and death, as well as property damage. These risks include, without limitation, exposure of the Participants to the risk of cuts and bruises, sprained or broken bones, torn ligaments and cartilage, concussions, and dislocations, viruses or other illnesses. Traveling to and from the airport, and within the country raises the possibility of any manner of transportation accidents. The Participant knowingly, voluntarily, and intelligently accepts and assumes responsibility for each and every risk and danger that could arise out of, or occur during the duration of the trip, including travel to and therefrom. The Participants expressly accept and assume responsibility for these risks and dangers whether they are known or unknown, or whether they are caused in whole or in part by the negligence, strict liability, or other acts or omissions of Followers Church, La Mision, or any of its affiliated organizations, officers, employees, directors, board members, volunteers, contractors, or agents.

The Participants hereby expressly represent that they are in good health and proper physical condition to participate in this trip: a) they have no physical, mental, or cognitive disabilities, ailments, or impairments that may affect their ability to endure travel, whether by motor vehicle or on foot; and b) they assume sole and full responsibility for determining the sufficiency of their health, fitness, and ability to utilize the attractions or participate.

In consideration for the Participants receipt of such good and valuable consideration, they fully and forever release and discharge Followers Church, its heirs, personal representatives, successors, assigns, officers, shareholders, members, agents, partners, employees, and attorneys from any and all actions, claims, causes of action, demands, or expenses for damages or claims, whether asserted or un-asserted, known or unknown, foreseen or unforeseen in any way related to or arising out of participation in activities before, during and after the trip, including travel to and therefrom, to the greatest extent allowed by law

IRS Regulation *

I understand that per IRS regulations and church policy, ALL contributions from ANY party (myself or others) on my behalf are non-refundable, even if I am unable to travel. All mission donations remain in an account for use by Followers Church for mission projects.

I understand that a non-refundable deposit of $250 is due within 30 days to reserve my spot on the team. *

Deposits can be paid in person by check or cash, or through Online Giving by designating the Guatemala Missions in the Fund category.

I am willing and able to raise the funds needed to cover the cost of this trip. *

I understand that I am responsible for obtaining the proper documentation (valid passport) and all required immunizations before traveling at my own expense. *

Any required immunizations will be communicated to me prior to travel.

I commit to follow the standards of conduct expected of all Followers Church volunteers and staff, to submit to the authority of the team leader(s) and to follow all team policies at all team events, as well as during travel and in country. *

While in country, I agree to submit to the leadership of our host(s) and to follow any additional rules and guidelines established for our and their safety as well as the preservation of relationships with the communities we will serve. *

I understand that if, at any time while in country, my behavior constitutes a problem for the team or for our Guatemalan hosts, Followers Church leadership and/or our host have the authority to require me to return home at my own expense. *

I commit to fully participate in all pre-trip and post-trip team meetings, team building and fundraising activities. Exceptions will be handled on a case-by-case basis. *

I am willing and able to give up certain comforts of home and live sacrificially while in country (including, but not limited to, use of tobacco, alcohol, food preferences, sleeping conditions, clothing preferences, etc.). *
